401(k) Plan

1st of month following 30 days of employment

Bank will match 100% up to 5% of salary. Immediate vesting. Pre-tax contributions

Employee Stock Ownership Plan (ESOP)

Eligible after 1 year of service

Bank makes discretionary contribution. Graduated vesting.

Cafeteria Plan

1st of month following 30th day of employment

Pre-tax premiums the full calendar year.

Long Term Disability

1st of month following 30th day of employment

Coverage is provided by the Bank for the Employee.

Group Term Life

1st of month following 30th day of employment

This is provided by the Bank. Coverage is equal to 3 times annual salary not to exceed $50,000 face value.

Dental

1st of month following 30th day of employment

 

Hospitalization

1st of month following 30th day of employment

 

Vision

1st of month following 30th day of employment
