IN CASE OF ERRORS OR QUESTIONS ABOUT YOUR ELECTRONIC TRANSFERS OR DIRECT DEPOSITS:

Write us at the address or call us at the telephone number shown above as soon as you can, if you think your statement or receipt is incorrect, you

 must contact us no later than  sixty (60) days after we sent you the FIRST statement on which the problem or error appeared and advise us of the

 following:

(1)     Your name and account number or present the statement on which the error occurred.

(2)      The error or transfer you are unsure about, and explain as clearly as possible why you need additional information.

(3)     The dollar amount of the suspected error.

If you tell us orally, we may require that you send us your complaint or question in writing within ten (10) business days.

We will correct any error promptly, but, if we need more time, we may take up to forty-five (45) days to investigate your complaint or question.  If we find the need  to do this, we will recredit your account within ten (10) business days for the amount you think the account is in error, so that you will have the use of the money during the time it takes us to complete our investigation.  (If we ask that your complaint or question be put in writing, and we do not receive it within ten (10) business days we may not recredit your account.)

After we finish our investigation, if we decide that there was no error, we will send you a written explanation within three (3) business days.  Copies of the documents used in our investigation may be requested by you.

Please examine this statement and cancelled checks promptly.  Your have fourteen (14) days to report unauthorized or missing signatures or alterations of the items contained with your statement.  If you fail to notify us, we will not be responsible for other items paid in good faith.  If no errors or discrepancies concerning Electronic Funds Transactions are reported within sixty (60) days, all such transactions will be considered correct.
